You can apply a predefined Form Template to a Form. You can also add Content to the Form Template. The following types of Templates are available:
1 Column: The Form is arranged into a single column.
1 Column Left with Content: The Form is arranged into one column and two Content placeholders are available to the right of the Form.
1 Column Right with Content: The Form is arranged into one column and two Content placeholders are available to the left of the Form.
1 Column Middle with Content: The Form is arranged into a single column at the center and one Content placeholder each is available on either side.
Click the Forms & Rules tab.
OR
Click the Sequences tab and expand a Sequence by clicking .
Click the Design button. Forms Builder changes to the Design mode.
Click a Template. The template layout is displayed on the Canvas.
Move a form from the Forms & Rules tab or from the Sequences tab on to the Form area of the Template.
The image below depicts the Online Application - Personal Information form being moved to the Template.
When you drop the form on to the Template, the Form fields are displayed.
Click SAVE. The Form is saved with the selected Template.
